How to get started

The first step is for you to print and fill out the request for appointment form:
I will need this form filled out and signed in a blue or black pen
One request for appointment form is needed for each person I will search for:

Secondly, I will need a legible copy of your drivers license, a copy of your birth certificate,
and any documents you may have pertaining to the adoption.   

Fill out and include the three forms below:

Please fill out and send in the Interview Questions form: 
(Please leave blank if you are unable to answer the question this form is designed for all members of the adoption triad and no one person can answer all the questions.)
 

New Client Form

This form needs to be signed dated and notarized.This form gives me Consent to Release Information to the biological relative I am looking for.

On this form please put what information you would like me to release to your biological relative.
When exchanging identifying information you will need to include your full name, date of birth and address.

Along with the above forms I will need your  $250.00 payment prior to me starting to actively work on your case.

Once the payment and paperwork is processed I go to the court to view your closed adoption file and find the name of the person you are searching for and begin searching for them.

Keep in mind an affidavit of no contact can be placed in the file preventing a Confidential Intermediary from conducting a search. If the court file contains an affidavit stating the person wishes no contact ever be made then I am unable to continue the search. It is rare to find an affidavit in a court file .In the event an affidavit of no contact is found in the file a refund will be issued on unused money. In some circumstances, the affidavit has been found in the file but the odds of finding one are low.
